New Patient Registration

Dr_WearneIf you live within our practice area and wish to register as a patient at the surgery please ask at reception for a new registration form which you will be asked to complete. We will also ask you for proof of your address e.g. utility bill or driving license.

An appointment will be made for your new patient health check which will be done by one of our practice nurses.

We have a defined practice area. If you move outside that area, you will need to register with another GP, even if the new address is relatively close to the old one. We appreciate the problems this occasionally causes, but we need to be fair to all patients.

Consulting Your Doctor

You will be registered with the practice as a whole but can express a preference to be treated by one of our GPs and that will be recorded.  You may however have to consult any available GP at busy times.

If you have not attended a consultation or clinic with the practice for three years or more you have the right to ask for and attend a consultation.

If you are aged 75 years or over you have the right to ask for a consultation every 12 months if you have not otherwise had one and this may be at your home if your medical condition requires it.
